For Financial Assistance from the Gladstone & District Sports Foundation Trust.

Each application for assistance will be assessed using the information provided against the Selection Criteria.

Applications may be lodged at any time of the year, however must be received a minimum of 14 days prior to competition for assessment purposes. If the application is successful, funds are paid retrospectively.

I have read and understand the Gladstone and District Sports Foundation’s Drugs in Sport Policy (see Guidelines & Information) which is based on the policy of the Queensland Government.

I understand, upon an application being successful, that funding is paid retrospectively and I am required to submit receipts from the event within one month of the event completion date.

Photographic permission

I hereby give the Gladstone & District Sports Foundation Trust, and those acting within its authorisation the right and permission to copyright, use and/or publish photographs and/or video of the participant in promotional or informational materials. I understand that the definition of publication that relates to this agreement includes any form of digital or non-digital media, including the internet, videos, catalogues, data retrieval systems, newspaper and magazine articles, brochures, pamphlets, displays and various hard copy publications. I verify that by signing this agreement, I waive any right to inspect or approve the finished videos, photographs, advertising copy or printed matter that may be used in conjunction herewith or to the eventual use that might be applied. I also approve the use of my personal information (listed above) on the understanding that it will be used for storage retrieval; referencing of photographs and documents; further consultation if needed; and captioning published images. I have read the foregoing photographic release and authorisation before affixing my signature below and warrant that I fully understand the contents thereof.
